Well, you got all the info for the ports.  Now is it possible that you are not putting the correct Host IP?  If this DSL router works like other routers, find your computers IP and add it to the Host IP.

If you have several or two or more computers, that router is the DHCP host, your computers are DHCP clients and will receive an IP that the router will asign to that computer.  To find out each computers IP, log in to that computer, go to Start, Run and typ in CMD.  At the blinking curser, type ipconfig /all

I think you would do it like this:
first you need to find out your pc's ip address as Tk said, lets say its 192.168.1.10.
then you need to enter 3100 in each of the 3 port boxes, check tcp and then 192.168.3.10 in the host ip address box.then add, and save.
now make a new line, line 2 . enter 3999 in the port boxes check tcp and enter the same ip in the ip box, now make line 3 use 3900 for ports, tcp ip the same. now do the same for ports 3100,3900, and 3568 but check udp , same ip address

1 3100   3100  3100  tcp  192.168.1.10
2 3999   3999  3999  tcp  192.168.1.10
3 3900   3900  3900  tcp  192.168.1.10
4 3100   3100  3100  udp 192.168.1.10
5 3900   3900  3900  udp 192.168.1.10
6 3568   3568  3568  udp 192.168.1.10

Should be able to range those.
1 3100   3399  dont enter  tcp  192.168.1.10 <--like that Leaving the private port blank. Try it?

I'm with TK.  Range the ports and you only need two lines.  One for TCP and one for UDP, unless you can select both check boxes, then you would only need one line.
